How we check it
- Every value on this site comes from the official source listed on the page and links back to it.
- A listing is published only if it has at least 3 of these attributes populated from the source: Specialty license, Sedation permit, Licensed for, Discipline record, License expires.
- Each record shows the date we last verified it against the source. Records that disappear from the source are removed at the next refresh.
- No text on listing pages is written by hand or generated without data: summaries restate the source fields.
- The cost calculator multiplies your inputs by cited rates and shows a low-to-high range, never a single price. Each rate links to its source with the date we checked it. Estimates are not quotes.
- Listings come from the source data alone: no provider pays to appear on this site, and it collects no personal information. The site has no request or contact form.
- This site loads no third-party scripts or fonts and sets no cookies. It counts page views and the use of its calculator and outbound links as daily totals, with no identifiers.
Data last verified: Oct 20, 2026.
